Failed to save Application Settings

i added “Comment Lines” button to the toolbar
but when was closing TOAD got this warning message “Failed to save Application Settings”.
Any clues how to save it?

can you clarify what do you mean about ‘added “Comments Lines” button’?
Actually I don’t understand what does it mean.
