Problem with Index Monitoring in 12.1.0.6

When selecting as schema from the Database\Monitoring\Index Monitoring I receive the message “Format ‘(%s)’ invalid or incompatible with argument.”. Log file below. I see similar behavior in other areas of the application if I need to select a schema such as Database\Monitoring\SGA Trace/Optimization.
