Every time i try to create a new connection to hive, get the below error

I have read the logs and found it was because mysql_upgrade.exe failed to execute. Could you please help to find the err file called <your_machine_name>.err. it is located at C:\Users\prabhupr\AppData\Local\Quest Software\Toad Data Point Hub Data\data\xxx.err,

after you have send the err file, could you help to have a try:
make sure TDP is closed and then go to rename the folder: C:\Users\prabhupr\AppData\Local\Quest Software\Toad Data Point Hub Data to a different name, e.g. Toad Data Point Hub Data_old.
start TDP, then try to connect to apache hive, to see if you can connect successfully.
if you are still not able to connect, please go to the newly generated folder: "C:\Users\prabhupr\AppData\Local\Quest Software\Toad Data Point Hub Data" and then collect the following two files:
C:\Users\prabhupr\AppData\Local\Quest Software\Toad Data Point Hub Data\data***.err
C:\Users\prabhupr\AppData\Local\Quest Software\Toad Data Point Hub Data\logs\guardian_xxxx.log
