Ctrl-T results in access violation

All,

I hit Ctrl-T to bringup a list of routines available in the package. In stead I got an access violation.

If this is not the right place to report this kind of issue, I apologize for posting.
If so would you please let me know where/how to report technical issues like these?

So here is the pop-up I with error info that I got:

I have captured the details too, but it seems a bit much to put that as text into this mail.

Would you please let me know how to proceed?

The issue occurred on an old version of Toad.

I was trying to do too many things at once, apparently.
