I have Toad set up to do nothing on start up. I would assume that means just
connect and do not open an editor window but when it starts, I have one default
open editor window.
Now if I select Open Editor on start up
I get two open editor windows.
What am I missing?

Toad has one more general option set to open Editor on new connection (Options|Database|General).
This is why you have one default open editor window.
Uncheck one of these options to get only one editor window on start up.
