How to customize the header in the Formatter

Hi,

I would like to know how to have a custom header format. Instead of the standard.

Standard: /* Formatted on 11/9/2018 2:09:03 PM (QP5 v5.326) */

Custom: /* Header: ProcedureName.sql 120.001 11/09/2018 02:09:03 OSUSER noship */

How can I achieve this. Is there an instruction manual where I can learn how to set this up.
