Verify that your options are set to include the object types you're interested in. See View|Options|Editor|Code Assist|"Toad Insight Options." Does it fail to work with all SQL or only certain ones? In a blank editor try CTRL+T. Do you get the list then?

After uupgrading to 12.1.1 code insight is not pop up with table columns in sql editor. I have tried also +T to manually bring it.
