This morning I started TOAD and it restored a workspace that had files open that were on a share that was not accessible at the time.
This caused all files from that share to show as "(Deleted)". After connecting to the share they were marked as "(Deleted) (Modified)". 
However, an editor tab is also displayed as "(Deleted)", which was never saved.
